Our program.  We will work with the older adult’s primary-care physician to assess common problems including:

• Memory loss
• Medication management
• Falls
• Decreased ability to live alone
• Depression
• Behavior changes
• Poor nutrition   

Clinical expertise.  Every Signal assessment is performed by an interdisciplinary team of medical experts.  The team includes a geriatrician, an advanced practice nurse, a social worker, a dietician, and occupational and physical therapists.  Comprehensive assessments include a medical and functional evaluation, as well as an in-home visit to ensure every aspect of wellness is considered when developing a care plan.  

Family support.  A strong support system is essential to an older adult living with a complex health issue.  The Signal team is skilled in providing family members the information needed to make educated decisions to help the older adult live the healthiest and most independent life possible.  Whether an assessment leads to changes in the home or a move to a senior living community, the Signal team is always available to provide support and encouragement.
